the conditions under which commissions are not earned or the various --------------------------------------
situations under which commissions can be forfeited or will require repayment
due to account cancellations etc.
I full understand that I am not an official representative or employee of
J&M TOWEL and that this program operates under an independent contractor
structure. As such, I acknowledge that my sales and referral activities will be
done at my own convenience, at my own expense, using my own equipment,
and at my own location(s). In accordance with I.R.S. requirements, I understand
that if my earned commissions meet and/or exceed $500.00 in a calendar year,
I will receive an I.R.S. form 1099 from J&M Towel stating the earned commissions
from my Sales Agent activities.
